question I am getting ready to join, but they ask for a cc card I have my netspend but don't always use it I use prepaid alot is that still ok, I know prob a stupid question. Some times I am airheaded...

Okay Ive been tryiong to send in the faxback form here for like 3 or 4 days now I have sent it to them 3 times now and they keep sending me a reply saying I havent filled the form out but I double checked and I went thropugh and made sure it was all done last time I sent it but still got the same reply I dont know what Im not filling out but last time they replied they said something about initialling in the checkboxes next to all the deposits Ive made but I havent deposited yet and there arent any checkboxes to initial in either so I dont know what to do any suggestions?>??
